Am in secret room 20, looked everywhere...am playing in easy mode, with visible enemies..

Is there something else I need to do before finding him? I want that Maddening Axe! Not finding it is...well...maddening!

maybe he is not available in easy mode?

The dwarf king only appears on normal and hard mode.

Nope, it's intended. Dwarf King (and the Maddening Axe) is simply a reward for playing one of the harder modes. And Indi mentioned once that the axe is not "needed" in Easy Mode, because the fights are easy enough already.
