can't find boss number 3

Ok i can't find the stupid number 3 boss in the swap .... isn't he suppose to be in that crypt place in the black cofifn with a skeleton on it ? Does that crypt really has no boss or enemy at all ?
If not ... then pls tell me where is this stupid boss number 3 ?

hmmm... besides unlocking New game + if you finish maniac mode .... is there any other in game benefit if you want to have a perfect score ? Or it's just for your personal ego ?
By the way what does new game + changes besides having all characters ?
