Page 51 of the guide says you should have sidequest #16 before entering the volcano.
This is a mistake. It should be after.

The guide says talk to Brett-Rhys for Q35, but that is the person who gets you to the bottom of the mountain.

The person with whom you need to talk is Carla for Q35.

Quest 48 states you need to finish 46 first. I believe that should read finish Quest 47 first.

I believe I fixed that already if you'd like to redl the guide.

Quest 40 (Mangrove fix) says you must have completed quest 28 first, but I think maybe this should be 29 (Cockaaa)? I have completed 28 but not 29, returned with Penelope, and can't get 40

Yep, I believe the latest download has this fixed.
The error originally comes from a quest being swapped from place 40 something to 25 (the sword in the earth quest).
After that I forget to shift all the other numerical references to quests
